POSITION SUMMARY

The RN Oncology Case Manager utilizes a collaborative process of assessment, planning, implementation and evaluation, to engage, educate, and promote/influence members decisions related to achieving and maintaining optimal health status.

Fundamental Components:

The RN Oncology Case Manager will: 1. Assess the member and family 2. Plan for their needs 3. Facilitate their care coordination 4. Advocate for options and services

BACKGROUND/EXPERIENCE desired:

3-5 years clinical oncology practice experience

Bilingual preferred

EDUCATION

The minimum level of education desired for candidates in this position is a Associate's degree or equivalent experience.

LICENSES AND CERTIFICATIONS

Nursing/Registered Nurse (RN) is Required

Nursing/Certified Case Manager (CCM) is desired

Nursing/Oncology Certified Nurse is desired
