Aetna Case Manager in Blue Bell, Pennsylvania

The RN Case Management utilizes their skills to collaborative assess, plan, facilitate care coordination, and advocate for options and services to meet an individuals and familys comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes.

Hours: Monday-Friday 9-5:30pm; 1 afternoon 12:30pm-9pm. This role could be Monday-Friday 12:30pm -9pm also. No weekends & holidays off. Must work on site and no telework.

Fundamental Components but not limited to:

The RN Case Management will:

  1. Assess the member and family

  2. Plan for their needs

  3. Facilitate their care coordination

  4. Advocate for options and services


    3-5 years clinical (med surg, ICU, telemetry) hospital practice experience

    Multiple State license will be required to be obtained upon employment

    Bilingual a plus

    HHC or Case Management experience preferred


    The minimum level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.


    Nursing/Registered Nurse is required

    Nursing/Certified Case Manager (CCM) is desired

    Functional - Nursing/Discharge Planning/1-3 Years

    Functional - Nursing/Home Health Care Ambulatory Nursing/1-3 Years

    Technical - Desktop Tools/Microsoft Word/4-6 Years/End User

    Position has been approved - replacement position - RN (preferred) with 3-5 years experience in a clinical setting with sound clinical assessment skills
