POSITION SUMMARY

The RN Case Management utilizes their skills to collaborative assess, plan, facilitate care coordination, and advocate for options and services to meet an individuals and familys comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes

Fundamental Components:

The RN Case Management will: 1. Assess the member and family 2. Plan for their needs 3. Facilitate their care coordination 4. Advocate for options and services

BACKGROUND/EXPERIENCE desired:

5 years clinical practice experience

CCM preferred

Bilingual a plus

EDUCATION

The highest level of education desired for candidates in this position is a Associate's degree or equivalent experience.

LICENSES AND CERTIFICATIONS

Nursing/Registered Nurse in PA is required

Nursing/Certified Case Manager (CCM) is desired

FUNCTIONAL EXPERIENCES

Functional - Nursing/Medical-Surgical Care/4-6 Years

Functional - Nursing/Home Health Care Ambulatory Nursing/1-3 Years

TECHNOLOGY EXPERIENCES

Technical - Desktop Tools/Microsoft Outlook/1-3 Years/End User

Technical - Desktop Tools/Microsoft Word/1-3 Years/End User

REQUIRED SKILLS

Telework Specifications:

Considered only for current internal teleworkers but must live in the Blue Bell PA area and an option for external candidates after a certain period of employment and performance
