Aetna Case Management Coordinator in Boynton Beach, Florida

POSITION SUMMARY

As a Case Management Coordinator you will manage caseloads for the Medicaid Long Term Care population in Palm Beach County. You will complete initial and routine comprehensive assessments for the purpose of identifying member care needs. You will also engage members routinely telephonically and in person through home/facility visits to coordinate services and community resources.

Fundamental Components:

Must have excellent communication, organization and time management skills. Must have Microsoft Office Suite and general computer knowledge

BACKGROUND/EXPERIENCE desired:3-5 years of Case Management experience. Long Term Care Diversion Program experience is preferred. Florida Department of Elder Affairs 701B Comprehensive Assessment Certification required. (Candidate can complete course to receive certificate upon hire) Willingness to travel within designated geographical region as required. Bilingual (Spanish and/or Creole) preferred.

EDUCATION

The highest level of education desired for candidates in this position is a Bachelor's degree or equivalent experience in Social Work. Master's degree preferred.

Telework Specifications:

WAH: employee will primarily work from home office but will come into the local office for meetings. Field visits can consist of visit to member's home, place of residence, local hospitals, and physician offices.
