This is an office based role in Chantilly, VA. A current internal teleworker can remain working from home if they are based in the state of VA.

The required hours are as follows: Monday-Friday, 2-3 days/week working from 11:30am-8pm with the remaining days during the week 8am-5pm. An occasional Saturday is required from 9am-2pm.

POSITION SUMMARY

The Health Coach Consultant utilizes a collaborative process of assessment, planning, implementation and evaluation to engage, educate and promote/influence member's decisions related to achieving and maintaining optimal health status.

Job Responsibilities

-Apply data driven methods of identification of members and be accountable to follow appropriate procedures for identifying, screening and assessing member needs

-Conduct telephonic individualized clinical assessments of member's health concerns/needs and develop a personalized condition-specific action plan

-Provide information to deepen their understanding of their medical condition and to guide the person in self- management -Screen members to identify opportunities for health services, modifiable risk factors and educational needs and provide appropriate partner referrals

-Intervene to positively affect healthcare outcomes

-Promote internal and external communication to enhance effectiveness of medical management services

-Reinforce appropriate education and monitoring; provide up-to-date health care information to help facilitate the members understanding of their health

-Assist members to be informed and actively engage with their provider in making the best health care decisions

-Evaluate the member's progress in setting and meeting established goals and revise their individualized care plan accordingly

BACKGROUND/EXPERIENCE

-Requires an active, unrestricted RN license in the state of Virginia

-RN with 4-6 years of clinical experience is required

-Disease Management experience required

-Motivational Interviewing is preferred

-Excellent verbal and written communication skills

-Proficiency in use of computer applications and typing skills (>35 wpm)

-Comfortable working in a call center environment

EDUCATION

The minimum level of education desired for candidates in this position is a Associate's degree or equivalent experience.

LICENSES AND CERTIFICATIONS

Registered Nurse (RN) is required

Health and Fitness/Wellness Coach is desired

Facilitate positive behavior change for members to improve their health. Communicate, problem solve and work effectively with people; use good judgment in new and challenging situations. Utilize motivational interviewing techinques, active listening skills, the Transtheoretical Model, mindfulness based and other evidence-based coaching techniques. Use your clinical decision making, coaching and motivational skills to support our members. Work within a multidisciplinary team. Full-time position; work hours vary from 8:30 am - 8:00 pm Monday-Friday and 9:00 am - 2:00 pm on Saturday; 3 evening shifts per week and occassional Saturday shifts are required.
