With a focus on delivering comprehensive, patient-centered care across the health care continuum, the Clinical Pharmacist will work closely with the JV clinical leadership, Physician Practices and Case Managers to develop and promote strategies and policies that enhance quality of pharmaceutical care in a cost effective manner to establish competitive business advantage
Assesses members medication regimens for indication, effectiveness, safety and adherence issues
Consults with providers, care managers and one-on-one with high risk members - Identify, resolve and prevent medication-related problems
Utilizes clinical and analytical skills to positively impact the health of members while helping to reduce health care costs
Participates in patient Care Conferences and contributes to the patient Plan of Care
Collaborates with the JV Medical leadership and Multidisciplinary Care Team in developing treatment guidelines and demonstrates the impact of these guidelines. May participate in formulary development and management
Monitors prescribing habits of physicians prescribing for JV patients. Makes recommendations to physicians to improve medication regimens as appropriate
Manages pharmacy component of medical benefit for assigned customers
Establishes and maintains external relations. Participates in national, regional and local workgroups, as needed
Serves as first line resource for pharmacy issues for the JV Partner.
Serve as a drug information resource for providers, Multidisciplinary Care teams and members
Consults in quality management initiatives
Assesses customer needs aligning with JV Strategy
Ensures effective communications between customers and business partners
Develops locally driven programs and products and evaluates effectiveness
In-depth knowledge and experience in managed care / managed pharmacy / competitive environment.
3-5 years clinical pharmacy/administrative experience or equivalent.
The highest level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.
LICENSES AND CERTIFICATIONS
Pharmacy/Licensed Pharmacist is desired
ADDITIONAL JOB INFORMATION
If tactically warranted and logistically possible, participate in practice patient care strategy meetings (huddles) as appropriate either locally or remotely.
When appropriate, conduct at-home assessments and intervention sessions with highest risk patient population.
Consults with Medical Director and/or other appropriate programs/resources to overcome barriers to meeting goals and objectives and presents cases to appropriate resources to obtain multidisciplinary view in order to achieve optimal outcomes.
Maintain a comprehensive working knowledge of community resources, payer requirements and network services for target population to maximize benefit to patient overall wellbeing.
Attend meetings as required.
Local travel is required
