OPOSITION SUMMARY

This Field Case manager role will be in the Lansing, MI area (there was not an option to post in Lansing, MI!) OR can include the Grand Rapids area as well!

The Registered Nurse Field Case Manager will work based from home and will include daily travel around a multi-county area to facilitate client participation in treatment plans towards full rehabilitation. The Case Manager coordinates and attends appointments with clients and providers, problem solves clinical issues as well as maintain ethical standards in all case management activities.

Fundamental Components

  • Requires a valid State of MI Registered Nurse license, is without open formal complaints or disciplinary action.

  • Must reside in or within 25 miles of Lansing, MI.

  • Must have excellent communication skills including ability to problem solve with others.

  • Requires strong knowledge of clinical protocols and ability to problem solving clinical issues

  • Requires organizational skills including ability to independently prioritize issues and coordinate own schedule

  • Be able to multi-task

  • Must be able to work before 8 am and after 5 pm weekdays and weekends as needed. Stipend provided for working Catastrophic referrals after regular business hours/weekends.

  • Requires strong technology skills including keyboarding, use of MS Office, and smartphones

  • Must have a reliable car and be willing to travel 100 - 200 miles per day, 5 days per week. Mileage reimbursement provided.

  • Must have a secure workspace at home to work uninterrupted, access to high speed internet, and ability to have private conversations without violating HIPPA standards.

    About 50% of time spent traveling and attending appointments with clients and medical providers to review medical status and ensure provider is following clinical protocols per the diagnosis. 25% of time spent arranging, facilitating and confirming client participation in appropriate treatment plan. 25% of time spent documenting activities and making recommendations.

    BACKGROUND/EXPERIENCE

  • Valid State of MI Registered Nurse license for at least 5 years and be in good standing.

  • Strong Clinical Nursing knowledge

  • Certified Case Manager (CCM) preferred but not required.

  • Must have valid Drivers License without any restrictions.

  • Field Case Management or Home Health Care Experience preferred.

  • Experience with customer service

  • Strong skills working with technology including computers and smart phones.

    EDUCATION

    The highest level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.
