POSITION SUMMARY

The Care Manager Coordinator may perform various computer-based assessments on LTSS members, and provide care management and care coordination in order to meet that member's needs. Main function for these 2 positions is for authorization department and for providing support to field CMs. Previous authorization experience or medical coding experience.

Fundamental Components:

-Care Management Coordinator role MAY include completion of NJ Choice assessment, additional computer documentation, and authorization of specific services to help members remain in the community as long as possible. Definite role includes completion of supportive tasks for the field Care Managers, and an additional focus on creating and updating authorizations, processing faxed requests for service authorization, as assigned.

BACKGROUND/EXPERIENCE:

-Home health, PCA agency, or NJ DMAHS or MCO Care Management background.

-Case management and discharge planning experience preferred

-Managed Care experience preferred

-Self-starter and good time management skills a must.

-Authorizations or medical coding experience preferred.

EDUCATION

The highest level of education desired for candidates in this position is a High School diploma, G.E.D. or equivalent experience.

Telework Specifications:

Flexible telework / office-based schedule with at least 6 weeks in office for training, then bimonthly in-office meetings. Otherwise work from home office.
