POSITION SUMMARY

The clinical care manager will provide face to face/telephonic education to members as appropriate. You would need to be in the Philadelphia, Erie or Pittsburgh areas.

You will have the opportunity to utilize advanced clinical judgment and critical thinking skills to facilitate appropriate member physical health and behavioral healthcare through assessment and care planning, direct provider coordination/collaboration, and coordination of psychosocial wraparound services to promote effective utilization of available resources and optimal, cost-effective outcomes

This is a PART TIME Role, perfect for someone looking to work 4 hours a day, 5 days a week will be needed, the schedule can have some flexibility as long as consistency is maintained. It will involve up to 50% local travel.

If you live close enough to the office to commute, you will be expected to work from the office when not traveling. If in the Erie area, you may need to come to the office for some training, yet will mainly be work from home.

Fundamental Components:

Working with the Medicaid population, to set goals around physical needs, and working with external services and family members. You will conference with other departments. The goal is short term case management for these members You will have a minimum of 4 face to face visits per month.

BACKGROUND/EXPERIENCE

RN with Active license or LCSW Required3-5 years of direct clinical practice experience , e.g., hospital setting or alternative care setting such as ambulatory care or outpatient clinic/facility (Med/surg and a pediatrics background would be an ideal match!) Behavioral Health along with Med/surg or pediatrics is a plus! Home health can be a plus too! Case management and discharge planning experience preferred CCM certification Preferred

EDUCATION

The highest level of education desired for candidates in this position is a Associate's degree or equivalent experience.
