POSITION SUMMARY

The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individuals benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes. Requires an RN with unrestricted active license and computer proficiency.

Fundamental Components:

This position requires a Registered Nurse with a minimum of 3 years clinical experience, 1 year case management, and discharge planning experience. Success in the position requires sound clinical assessment and decision-making, professional judgment, excellent verbal and written communication, teamwork, strong customer service skills, and computer literacy (Microsoft Outlook, Excel and Word). Must be willing and able to rapidly become an expert in the Lockheed Martin benefit plan and an integrated model.

Evening hours Mon-Fri till 8pm on a rotating basis with the potential for 10 hour shifts.

BACKGROUND/EXPERIENCE desired:

Pediatric or Oncology experience is highly preferred

EDUCATION

The highest level of education desired for candidates in this position is a Associate's degree or equivalent experience.

LICENSES AND CERTIFICATIONS

Nursing/Registered Nurse (RN) in OH is required

Nursing/Certified Case Manager (CCM) is desired
