POSITION SUMMARY

The Wellness/Health Nurse Coach utilizes a collaborative process, in conjunction with the client's health and wellness staff, to plan and implement wellness events and programs to promote/influence members in decisions related to achieving and maintaining optimal health status. The goal of these programs/events is to help members achieve healthy lifestyle behaviors and align these lifestyle behaviors with individual wellness goals.

Fundamental Components:

Role will be split between lifestyle health coaching support and disease management nurse coaching support, biometric screening support, presentations and events to be delivered at client location. Work at home with travel to client locations up to 5 days/week required. Topics to be delivered can focus on weight management, stress management, tobacco cessation, nutrition, physical activity and other healthy lifestyle topics and chronic conditions. The goals of wellness promotion programs is to help members understand and develop awareness of healthy lifestyles and align lifestyle behavior with personal wellness goals.

BACKGROUND/EXPERIENCE desired:

3-5 years of clinical and/or coaching experience. Experience in wellness related areas such as weight management, tobacco cessation, stress management, nutrition, exercise, chronic condition management, behavior change. Background in motivational interviewing, active listening skills, other evidence-based coaching techniques preferred. Experience building a culture of health in the workplace.

EDUCATION

The minimum level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.

LICENSES AND CERTIFICATIONS

Nursing/Registered Nurse is required

Health and Fitness/Wellness Coach is desired

Health and Fitness/Health Fitness Specialist Certification is desired

Nursing/Certified Dietitian/Nutritionist is desired

Telework Specifications:

Telework is required for this position, no AHM office location. Nurse will be required to visit client location in Savannah, GA 5 days/week.

ADDITIONAL JOB INFORMATION

Three to five years clinical experience with healthy lifestyle coaching for weight management, tobacco cessation, nutrition, physical activity, stress management, chronic condition management with experience in delivering coaching in an onsite client setting. Experience in facilitating patient/ client positive behavioral change Demonstrated ability to communicate, problem solve, and work effectively with people, use good judgment in new and challenging situations. Background in motivational interviewing, active listening skills, Transtheoretical Model, mindfulness based and other evidence-based coaching techniques preferred. Experience in developing and leading onsite wellness classes- requires strong skill set in public speaking/presentations Strong clinical decision making skills. Strong desires to coach, motivate, and support members. Ability to work with a multidisciplinary team. An understanding of management of chronic health conditions, especially as it relates to Diabetes, Obesity and Hyperlipidemia. Excellent computer skills including MS Office applications such as Word, Outlook, PowerPoint.
