A Clinical Care Coordinator works using a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual's and family's comprehensive health needs through communication and available resources to promote quality cost effective outcomes. You will authorize comprehensive home and community based and institutional healthcare and behavioral health services to facilitate delivery of appropriate quality healthcare, promote cost effective outcome and improve program/operational efficiency involving clinical issues. You will be able to exercise independent decision making skills.
This role will be office based the first year and could turn into a flex schedule after a year including 4 10 hour shifts!

Fundamental Components:Case management for long term care members who reside in Pima County.Travel to members homes and places of residence and complete assessments to ensure member safety, medical needs are met and services are provided.A clinical care coordinator will utilize his/her clinical knowledge to assess, plan, and implement services necessary to meet members health care needs.
BACKGROUND/EXPERIENCE 2-5 years clinical practice experience, e.g., hospital setting, alternative care setting such as home health or ambulatory care required including Behavioral Health.Case management and discharge planning experience preferredManaged Care experience preferredStrong time management/organizational skills, independent thinking, ability to work in a team environmentMS Office: Word, Excel and Outlook are neededLicensure: RN with Behavioral Health experience, LCSW, LPC, LMSW (preference would be the LCSW, LPC or RN)
EDUCATIONThe highest level of education desired for candidates in this position is a Master's degree.
LICENSES AND CERTIFICATIONSOne of the following is required!
