POSITION SUMMARY

The RN Case Management utilizes their skills to collaborative assess, plan, facilitate care coordination, and advocate for options and services to meet an individuals and family's comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes.

Hours for this position are Monday-Friday 9am-5:30pm EST with an Evening Shift rotation from 12:30-9pm EST 1-2x per week.

Fundamental Components:

The RN Case Manager will 1. Assess the member and family 2. Plan for their needs 3. Facilitate their care coordination 4. Advocate for options and services.

BACKGROUND/EXPERIENCE:

3-5+ years clinical practice experience; Required

Home Health Care or Case Management experience; Preferred

Bilingual; Preferred

EDUCATION

The highest level of education preferred for candidates in this position is a Bachelor's degree or equivalent experience.

LICENSES AND CERTIFICATIONS

Nursing/Registered Nurse is Required

Nursing/Certified Case Manager (CCM) is preferred
