This Registered Nurse Case Manager position is a great opportunity to join Aetna's National Account Care Management Solutions Team located in our Blue Bell, PA office. You'll have the opportunity to work in an exciting, fast pace atmosphere administering a high touch integrated care model for dedicated customers.
The Nurse Case Manager utilizes a collaborative approach in managing members health through assessment, planning, and facilitation of care, providing resources and support services resulting in optimal member centric care with cost-effective outcomes. The CM process includes but is not limited to assessing the member's health status and care coordination needs, review of inpatient events and discharge planning, developing and implementing case management plans, and Medical Director evaluation of case plans as indicated. The CM will possess solid communication and educational skills and have a strong knowledgebase of medical conditions and disease management. This position requires a RN license with an unrestricted active license.
3-5 years experience in an Acute care, Med/Surg, and/or Home Health setting required
Bachelor's degree preferred
Certified Case Manager (CCM) certification preferred
The highest level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.
