We are building an exciting new clinical and member experience program at Aetna. Aetna Community Care is a member centric, team-delivered, community based care management model that joins members where they are.
The Registered Dietitian is responsible for promoting optimal nutritional health, food safety and access, for high risk commercial and Medicare members in a new community based care management program. The Registered Dietitian, working within a multi-disciplinary team, leads and implements dietary strategies and interventions that support holistic member centered care along the health continuum.
Provides subject matter expertise, consultations, case reviews and education to clinical staff and members.
Participates in the inter-disciplinary team and collaborates with healthcare team to align nutritional interventions with care goals.
Evaluates the plan of care and effectiveness/outcomes of dietary interventions and modifies as needed.
Educates members and families on healthy diet, appropriate nutrition guidelines for disease state(s), provides prevention and wellness, guidance.
Connects members with nutritional community resources, dietary counseling services, leveraging applicable nutritional plan benefits.
Collaborates with nutritional therapy providers, home care providers, community resources for nutritional support.
Conducts home visits, as needed, to validate appropriate nutritional needs are being supported.
Utilizes evidence based knowledge and protocols appropriate to patient populations and diagnoses.
Adheres to care management and quality management processes in compliance with regulatory and accreditation guidelines and company policies and procedures.
Other responsibilities as assigned.
Background/ Experience Desired:
3-5 years related work experience as a Registered Dietitian required
Experience working long-term with people who have chronic conditions e.g. diabetes, renal disease, complex nutritional needs required
Hospital experience preferred
Demonstrates skills/knowledge needed to coordinate & support prescribed nutrition therapies
Build relationships & work collaboratively within an interdisciplinary team
Local travel may be required.
The highest level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.
Masters Degree in Nutrition or related field preferred.
Current Registration as a dietitian with the Commission on Dietetics in state where individual will work.
Position is to be located in Harrisburg, PA or remote in NJ.
