This is a telework role with 75-100% of travel around Cook County.

POSITION SUMMARY

This field-based position utilizes critical thinking and judgment to collaborate and inform the case management process in order to assist in facilitating appropriate healthcare outcomes for members. The position provides care coordination, support and education for members transitioning from skilled nursing care into the community through the use of care management tools and resources.

Fundamental components include but are not limited to:

-Outreach: Field-based position that provides in-person education and information to nursing facility residents and staff in Northern area of Cook County

-Member Evaluation: Conduct comprehensive evaluation of referred members needs/eligibility for transition to the community

-Care Plan Development: Identifies high risk factors and service needs for members

-Document Gathering: Assists in securing vital documents needed for transition

-Interdisciplinary Collaboration, Monitoring, Evaluation and Documentation of Care

-Using holistic approach consults with collateral contacts, supervisors, and/or other health programs to overcome barriers to meeting goals and objectives; identifies and escalates quality of care issues through established channels

BACKGROUND/EXPERIENCE

Bachelor's Degree is required

2 or more years experience in behavioral health, social services required

Managed Care experience preferred

Case management and discharge planning experience preferred

EDUCATION

The minimum level of education desired for candidates in this position is a Bachelor's degree.
