POSITION SUMMARY

The Wellness/Health Coach utilizes a collaborative process of assessment, planning, implementation and evaluation, to engage, educate, and promote/influence members in decisions related to achieving and maintaining optimal health status.

Fundamental Components:

Previous coaching experience preferred including face to face, 3-5 years of coaching experience.

Experience in wellness related areas such as weight management, tobacco cessation, stress management, nutrition, physical activity or health lifestyle behavior change.

Background in motivational interviewing, active listening skills, mindfulness based and other evidence-based coaching techniques preferred.

Experience in facilitating patient/client positive behavioral change.

Ability to work with a multidisciplinary team.

Excellent computer skills including MS Office applications such as Excel, Word and Outlook.

Demonstrated ability to communicate, problem solve, and work effectively with people, use good judgment in challenging situations.

BACKGROUND/EXPERIENCE desired:

3-5 years of coaching experience.

EDUCATION

The highest level of education desired for candidates in this position is a Associate's degree or equivalent experience.

Telework Specifications:

Considered only for current internal teleworkers (in any area), or onsite resources in Hartford, CT, Sugarland, TX, Sandy, UT, and High Point, NC. Option for telework will be evaluated after a certain period of employment

ADDITIONAL JOB INFORMATION

Provide recommendations for healthy behavior changes according to established protocols Establish plan of care based on members health status, and educational and support needs based on assessment Assist members in weight management, quitting tobacco, stress management, nutrition, physical activity and other healthy lifestyle behavior change programs Identify co-morbid conditions, hospitalizations, prescriptions, and other factors that could impact the coaching and support needs of the member outside the scope of the health coaching program Support clinical integration and collaboration on members care plan in conjunction with other health professionals Refer cases needing more intensive management when indicated to condition management, etc. Facilitate group meetings.
