POSITION SUMMARY

The Case Manager for the Healthcare Vertical team utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individuals benefit design and/or acute health needs through communication and available resources to promote optimal, cost-effective outcomes. Requires an RN with unrestricted active license.

Providing education to promote health engagement

Working closely with members to implement an individual plan of care

BACKGROUND/EXPERIENCE desired:

3+ years clinical practice experience is required

Case Management experience preferred

EDUCATION

The minimum level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.

LICENSES AND CERTIFICATIONS

Nursing/Registered Nurse (RN) is required

Nursing/Certified Case Manager (CCM) is desired
