POSITION SUMMARY

The Case Management Coordinator (CMC) will work in the ICM team and provide support and care coordination to members identified for care management. This position will collaborate with Case Managers, Care Management Associates, interdepartmental team members, provider partners, and community resource partners.

BACKGROUND/EXPERIENCE desired:

1+ years of Case Management and discharge planning experience is required

2+ years of experience in behavioral health, social services or appropriate related field equivalent to program focus is required

Must be able to work independently

Managed Care experience preferred

EDUCATION

The minimum level of education desired for candidates in this position is a Master's degree.

LICENSES AND CERTIFICATIONS

Mental Health/Licensed Social Worker (LSW) is required

OR

Mental Health/Licensed Professional Counselor (LPC) is required
