Responsible for the daily supervision of the Care Management supports staff. Serves as the subject matter expert to members, employees, and/or providers regarding general information about support programs for the KY 1115 Waiver. Plans, develops, and implements a comprehensive curriculum of programs for members, employees, and/or providers to promote and support support programs for the waiver plans. Serves as a liaison with internal departments, participating integrated delivery systems, and community resources to implement educational programs.

Fundamental Components:

This position will guide the support staff for the care management team. This includes support for the KY 1115 Waiver plan implementation and ongoing oversight of current programs in place. This will include outreach for identified members for completing of various tasks to include the state mandated health risk assessment and others.


Previous (3-5 years) experience in health education/wellness programs strongly preferred. Expertise in related disciplines such as dietary management or nutrition desired.

Previous supervisory experience preferred, but not required

Previous program management and evaluation experience required.

Demonstrated interpersonal and delegation skills required.

Excellent written and verbal communication and presentation skills.

Demonstrated analytical and problem-solving skills.

Managed care experience preferred

Ability to use standard corporate software packages and corporate applications.


The highest level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.
