Fundamental Components but not limited to the following:
The Workers Compensation Field Case Manager is responsible for assessing and analyzing injured employees to evaluate their medical and vocational needs. The Workers' Comp Field Case Manager must facilitate the patients appropriate and timely return to work, acting as a liaison between the patient/family, employer, providers, insurance carrier, and other healthcare personnel. This is a field-based position in Tifton, GA, and surrounding areas with expected travel of 75-100%. Travel is locally-based, and no overnight travel will be required. When not traveling, the candidate will be working from home. Applies all laws and regulations that apply to the provision of rehabilitation services; applies all special instructions required by individual insurance carriers and referral sources. Daily travel in the field up to 100 mile radius from your residence. This position requires one of the following national certifications: CCM, COHN, COHN-S, CRRN, CDMS and GA Rehab Supplier.
BACKGROUND/EXPERIENCE:
Minimum of three years case management experience required
One of the following certifications: CCM, COHN, CDMS, CRRN or GA Rehab Supplier required
3-5 years clinical RN practice experience required
Ability to travel within a designated geographic area for in-person case management activities
Bilingual preferred
Knowledge of laws and regulations governing delivery of rehabilitation services.
Job-specific technical knowledge, (e.g., knowledge of workers compensation and disability industry for workers compensation case managers or case management).
Excellent analytical and problem solving skills
Effective communications, organizational, and interpersonal skills.
Ability to work independently (may require working from home).
Proficiency with standard corporate software applications, including MS Word, Excel, Outlook and PowerPoint, as well as some special proprietary applications.
Effective computer skills including navigating multiple systems and keyboarding
EDUCATION
The minimum level of education desired for candidates in this position is a Associate's degree.
LICENSES AND CERTIFICATIONS
Nursing/ Registered Nurse is required
Most have a valid GA and FL RN License and one of the following certifications: CCM, COHN, COHN-S, CRRN or CDMS
Nursing/Certified Occupational Health Nurse is desired
Nursing/Certified Case Manager (CCM) is desired
Nursing/Certified Disability Management Specialist (CDMS) is desired
FUNCTIONAL EXPERIENCES
Functional - Nursing/Case Management/1-3 Years
TECHNOLOGY EXPERIENCES
Technical - Desktop Tools/Microsoft Outlook/4-6 Years/End User
