We are seeking self-motivated,energetic, detail oriented, highly organized, tech-savvy Registered Nurses tojoin our Workers Compensation Field Case Management team. Work from home position that includes daily travel in a multi-county area around Philadelphia, PA and Wilmington, DE areas to facilitate client participation in medical treatment plans towards full rehabilitation. The Case Manager coordinates and attends appointments with clients and medical providers, problem solves clinical issues, coordinates and facilitates treatment needs as well as maintains ethical standards in all case management activities.
Fundamental Components but not limited to the following:
Requires strong knowledge of clinical protocols and ability to problem solving clinical issues
Requires flexible work schedule including occasionally before 8 am and after 5 pm weekdays and weekends as needed. Stipend provided for working Catastrophic referrals after regular business hours/weekends.
Must have a reliable car and be willing to travel daily, 5 days per week.
Must have a secure workspace at home to work uninterrupted, have access to high speed internet, and ability to have private conversations without violating HIPPA standards.
About 50% of time spent traveling and attending appointments with clients and medical providers to review medical status and ensure provider is following clinical protocols per the diagnosis. 25% of time spent arranging, facilitating and confirming client participation in appropriate treatment plan. 25% of time spent documenting activities and making recommendations.
Case management or home care background preferred
5 years clinical practice experience preferred
Ability to travel within a designated geographic area for in-person case management activities
Bilingual preferred (Spanish speaking)
Knowledge of laws and regulations governing delivery of rehabilitation services.
Job-specific technical knowledge, (e.g., knowledge of workers compensation and disability industry for workers compensation case managers or case management).
Excellent analytical and problem solving skills
Effective communications, organizational, and interpersonal skills.
Ability to work independently (may require working from home).
Proficiency with standard corporate software applications, including MS Word, Excel, Outlook and PowerPoint, as well as some special proprietary applications.
Effective computer skills including navigating multiple systems and keyboarding
The minimum level of education desired for candidates in this position is a Associate's degree or equivalent experience.
LICENSES AND CERTIFICATIONS
Nursing/Registered Nurse (RN) is required
One of the following certifications are highly preferred and if not will need to be obtained within 2 years time:
Nursing/Certified Case Manager (CCM) preferred
Nursing/Certified Disability Management Specialist (CDMS) preferred
Nursing/CRRN - Certified Rehabilitation Registered preferred
Nursing/COHN - Certified Occupational Health Nurse is preferred
Functional - Nursing/Case Management/1-3 Years
Functional - Nursing/Critical Care/1-3 Years
Functional - Nursing/Occupational/1-3 Years
Functional - Nursing/Home Health Care Ambulatory Nursing/1-3 Years
Technical - Desktop Tools/Microsoft Word/1-3 Years/End User
Technical - Desktop Tools/Microsoft Outlook/1-3 Years/End User
