The Specialty Health Care Management (SHCM) program is managed by a team who reach out to patients filling medications at Aetna Specialty Pharmacy who are newly diagnosed with chronic diseases, are new to a drug/treatment, new to Aetna or have an issue with adherence/compliance to their specialty medications. Requires LPN with unrestricted active license.
Nurse will outreach to patients to offer the program and complete telephone assessments for patients who agree to speak to a nurse. The nurses provide drug and disease education, copay assistance coordination, referral services, side effect management and adherence monitoring. Nurse will also outreach to patients who are non-compliant with medication therapy and address barriers to adherence. This position requires nursing skills and clinical judgment.
2 years managed care experience .
2 plus years clinical experience required
The highest level of education desired for candidates in this position is a Associate's degree or equivalent experience.
LICENSES AND CERTIFICATIONS
Nursing/Licensed Practical Nurse (LPN) is required
Functional - Nursing/Disease management/1-3 Years
Functional - Nursing/Drug Information & Safety/1-3 Years
Functional - Communications/Member communications/1-3 Years
Technical - Desktop Tools/Microsoft Outlook/1-3 Years/
