Excellent opportunity to assist children, ages 0 - 21 years to obtain physical exams and follow-up care and services when needed. Data analysis and interpretation and interactions with members, providers and staff throughout the health plan.
Educational outreach to members and providers to assist member to obtain well care and services and assist providers to understand the state of Pennsylvania's EPSDT requirements for members who receive insurance through the state's Medicaid program.. Responsible to obtain and analyze data that assists in understanding the targeted population. Requires onsite visits to provider offices in the state of Pennsylvania and attendance at various meetings throughout the state.
3+ years of coaching experience
Experience in wellness related areas such as weight management, tobacco cessation, stress management, nutrition, physical activity or health lifestyle behavior change and EPSDT requirements
Background in motivational interviewing, active listening skills, Transtheoretical Model, mindfulness based and other evidence-based coaching techniques preferred.
Experience in facilitating patient/client positive behavioral change
Ability to work with a multidisciplinary team
Excellent computer skills including MS Office applications such as Excel, Word and Outlook.
Demonstrated ability to communicate, problem solve, and work effectively with people, use good judgment in challenging situations
RN Licensure required
The highest level of education preferred for candidates in this position is a Bachelor's degree or equivalent experience.
