Responsible for designing and implementing a health deliver program that ensures the needs of a member in the health system are met, including housing, vocational, residential, and nutritional. Responsible for developing and maintaining strong relationships with community organizations and coordinating services between organizations to address the non-health care needs of our members. Responsible for overseeing multiple teams of Community Health Workers throughout the state of Pennsylvania.

Fundamental Components but not limited to the following:

  • Oversees the design and implementation of the health delivery programs to ensure the needs of clients and their families in the health system are met, specifically surrounding Housing, Vocational, Residential, and Nutritional needs

  • Coordinates with health plan staff (e.g. Member Services, Utilization Management, Care Management) to secure needed services for individual health plan members

  • Utilizes community resource expertise and alternate funding arrangements available for health plan members (waivers, grants) when services are not available under the health plans benefit program

  • Interact with clinical and seniors leaders within the health plan to discuss member specific housing, vocational, nutritional and service needs

  • Develops an action plan that promotes permanent housing, vocational and nutritional needs

  • Assists members with application submissions for relevant programs.

  • Provides training and support in an effort to provide continuous improvement in access to housing, food, vocational and residential services

  • Garner trust and collaboration between individuals and community organizations.

  • Develops staff skills and competencies through training and experience.

  • Leads by example to create desired culture and motivated staff

  • Establishes a positive organization and company image within the health care community.

  • Establishes and maintains ongoing relationships with key agencies, organizations, providers, physicians, hospitals, and advocacy groups

  • Manages, coaches and oversees a team of community health workers

  • Other duties as assigned


5+ years in clinical area of expertise preferred in the Pennsylvania local area.

3+ leadership experience required.

2+ years Project coordination or project management experience is required.


The minimum level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.

License or Certification one of the following preferred:
