Exciting opportunity to work independently, managing a case load of approximately 20-30 patients, assessing and coordinating physician-recommended treatment plans. Perform medical field case management within the workers compensation industry. Act as a liaison amongst patients, adjusters, employers, physicians, and other healthcare providers. Coordinate and assess medical care in order to promote healthy outcomes and safe return to work. Work from home with in-state travel.

Fundamental Components but not limited to the following:

Communicate and meet with patients, adjusters, employers, physicians, other healthcare providers, and attorneys in order to effectively ensure progress toward a positive medical outcome and safe return to work. Travel to and attend physician, physical therapy, occupational therapy, and other ancillary medical visits with the patient. Assess patient's medical status and symptoms to determine progress toward recovery and return to work. Meet with employers to assess availability of modified duty and perform job analyses of both regular and modified duty positions. May travel to perform in-home patient assessments. Track and enter billable time via company software on laptop or IPhone. Write and submit reports on each patient. Territory may include Auburn/Lewiston up to north of Bangor, and out to the coast. Travel down to Portland is also likely.



5 years clinical RN practice experience (ortho, rehab, home health, acute, or neuro)

Excellent analytical and problem solving skills

Effective communications, organizational, and interpersonal skills.

Ability to work independently.

Proficiency with standard corporate software applications, including MS Word, Excel, Outlook and PowerPoint, as well as some special proprietary applications.

Effective computer skills including navigating multiple systems and keyboarding


Prior case management or VNA experience preferred.

Ability to travel within a designated geographic area for in-person case management activities

Knowledge of laws and regulations governing delivery of rehabilitation services.

Knowledge of workers compensation and disability industry for workers compensation case managers or case management.


The minimum level of education desired for candidates in this position is a Associate's degree or equivalent experience.


Nursing/Registered Nurse (RN) is required

Nursing/Certified Case Manager (CCM) is desired

Nursing/CRRN - Certified Rehabilitation Registered is desired


Functional - Medical Management/Medical Management - Workers compensation/1-3 Years

Functional - Medical Management/Medical Management - Case Management/1-3 Years

Functional - Medical Management/Medical Management - Home health care/1-3 Years

Telework Specifications:

Position is home-based, but with regular in-state travel. Coventry provides laptop, IPhone, fax/scanner/printer, as well as office supplies.


Posting is for a full time, 40 hour per week Nurse Case Manager. I will consider 32 hours per week. Salary will be commensurate with skills/experience.

Mileage reimbursement is provided per the IRS rate. The position is salaried at a competitive rate with a monthly bonus available for work completed above minimum expectations.
