POSITION SUMMARY

The Health Coach Consultant utilizes a collaborative process of assessment, planning, implementation and evaluation to engage, educate and promote/influence member's decisions related to achieving and maintaining optimal health status.

Fundamental Components:

-Apply data driven methods of identification of members and be accountable to follow appropriate procedures for identifying, screening and assessing member needs

-Conduct telephonic individualized clinical assessments of member's health concerns/needs and develop a personalized condition-specific action plan

-Provide information to deepen their understanding of their medical condition and to guide the person in self- management -Screen members to identify opportunities for health services, modifiable risk factors and educational needs and provide appropriate partner referrals

-Intervene to positively affect healthcare outcomes

-Promote internal and external communication to enhance effectiveness of medical management services

-Reinforce appropriate education and monitoring; provide up-to-date health care information to help facilitate the members understanding of their health

-Assist members to be informed and actively engage with their provider in making the best health care decisions

-Evaluate the member's progress in setting and meeting established goals and revise their individualized care plan accordingly

BACKGROUND/EXPERIENCE:

-Requires an active, unrestricted RN license in the state of Virginia (prefer multi-state compact license)

-RN with 4+ years of clinical experience is required

-Disease Management experience required

-Motivational Interviewing is required

-Excellent verbal and written communication skills

-Proficiency in use of MS Office computer applications and typing skills (>35 wpm)

-Comfortable working in a call center environment

EDUCATION:

The minimum level of education required for candidates in this position is a Associate's degree or equivalent experience.

LICENSES AND CERTIFICATIONS:

  • Active unrestricted RN license in VA or multi-state, compact license required

  • Health and Fitness Coach Certification is desired

  • Candidate needs to be willing to apply for any additional RN licenses that may be required in order to work with members in different states and are outside of the compact or states where they currently hold licenses.

Facilitate positive behavior change for members to improve their health. Communicate, problem solve and work effectively with people; use good judgment in new and challenging situations. Utilize motivational interviewing techniques, active listening skills, the Trans theoretical Model, mindfulness based and other evidence-based coaching techniques. Use your clinical decision making, coaching and motivational skills to support our members. Work within a multidisciplinary team. Full-time position; work hours vary from 8:30 am - 8:00 pm, Monday-Friday and 9:00 am - 2:00 pm on Saturday; 2 evening shifts per week and occasional Saturday shifts are required.
