Aetna Care Associate Behavioral Health-Midnight shift in South Jordan, Utah

Job Description

Provide service to clinical teams in support of optimal care.

Schedule is Monday-Friday start 6pm-12/Midnight and end 6am mountain time. Saturday nights & weekends. Flexible schedule is needed with a floating day.

Fundamental Components included but are not limited to:

Assists members locate services. Facilitates follow-up calls. Collects information on patient progress and adherence to treatment plan. Provides information to members regarding mental health and substance abuse benefits, community treatment resources, mental health managed care programs, and Aetna policies and procedures. Reviews patient compliance with treatment plan; identifies obstacles to compliance; works with Care Managers to develop action plans. Follows-up with members regarding missed aftercare appointments. Follows-up with outpatient providers to assure timely outpatient follow-up appointments for patients discharged from the hospital. Serves as liaison between the inpatient staffs of local hospitals and high volume provider groups to assure timely discharge appointments. Participates in reporting activities including data collection, tracking, analysis and preparation. Activities may include QI initiatives, post discharge and recidivism follow-up reporting, review and analysis of quarterly recidivism reports for quality improvement reporting. Administers assessment tools that track patient symptom changes. Working on an inbound call center que 50% of the time.

Qualifications Requirements and Preferences:

1 year+ Behavioral Health experience working with mental health population.

EDUCATION

Associates degree; Bachelor’s degree in Psychology, Social Service, or Health Administration. Experience in lieu of degree.
